The circulatory system is composed of cardiovascular system including arteries, veins, capillaries which distribute blood and lymphatic system which distributes lymph. The blood is oxygenated when tiny blood vessels, called capillaries, absorb oxygen from the lungs and in turn discharge carbon dioxide, which we then exhale. At the cells the blood vessels once more become a network of extremely thin capillaries, which give off oxygen plus other nutrients and in return receive waste products. Oxygen plus glucose is needed to make energy within every cell of the body; the waste products are carbon dioxide and water.
The blood vessels that supply cells with nutrients and oxygen are called arteries, while the blood vessels that carry away waste products and carbon dioxide are called veins. Arterial blood is brighter red than venous blood because oxygen is carried on a complex called hemoglobin, which contains iron. The pressure in the arteries is also greater than in the veins. Returning to the heart after it has visited the cells, all blood passes through the kidneys. Here, waste products are removed, and formed into urine that is stored in the bladder.
The human heart maintaining the blood circulation of cardiovascular system is a chambered, muscular organ that pumps blood from veins into the arteries by repeated, rhythmic contractions. It is about the size of a fist between 250 and 350 grams. It is located between your lungs in the middle of your chest, behind and slightly to the left of your sternum (breastbone). The heart plays a vital role of life maintenance and meanwhile cardiovascular disease is the leading cause of death worldwide. Over 459,000 American deaths and 101,00 deaths in the United Kingdom annually are due to coronary heart disease. The WHO (World Health Organization) has reported that the number of disability and death due to cardiovascular disease in sub-Saharan Africa rose from 5.3 million for men and 6.3 million for women in 1990 to 6.5 million and 6.9 million in 2000, and could rise to 8.1 million and 7.9 million in 2010.
ATHEROSCLEROSIS-Disease of arteries and precursors of heart disease
Heart disease is wrongly named. The main life-threatening diseases are disease of the arteries. ASVD known as atherosclerosis is a medical condition that the arteries become hard, narrow and thick due to the build-up of fatty materials such as cholesterol. It is dangerous because it is a chronic disease which begins in early adolescence and usually remains asymptomatic. However, decades of fat deposition will form arterial plaque or atheroma which narrows the arteries, reduces the blood flow, increases blood pressure. Atherosclerosis, coupled with thicker than normal blood containing clots, can lead to a blockage in the arteries, which stops the flow of blood and leads to death of the tissues fed by the artery in approximately 5 minutes. If this catastrophic event occurs in the arteries feeding the heart, the part of the heart fed by these blood vessels will die from lack of oxygen. That is the myocardial infarction (heart attack). If a blockage occurs in the brain, the affected brain cells will die and thus cause stroke (see Brian Care Package). If a blockage occurs in the legs, it can result in leg pain, which is a form of thrombosis (a thrombus is a blood clot). When peripheral circulation, for example in the hands and feet.
When you get atherosclerosis in the arteries of heart, there will be insufficient blood supply to heart due to the narrowing or partial blockage of coronary arteries. Angina is a condition when the chest pain occurs under different moments like exertion, severe emotional stress, heavy meals or vigorous exercise during which the heart muscle demands more blood oxygen than the partial blockage of coronary arteries can deliver. It often lasts from 1 to 15 minutes and is relieved by rest or by placing a nitroglycerin tablet under the tongue. Nitroglycerin can relax the blood vessels and lowers blood pressure. Both rest and nitroglycerin decreases the heart muscles demand for oxygen, thus reliving angina. Angina is only the symptom of atherosclerosis and the sign of heart attack.
A heart attack (also known as a myocardial infarction) is the death of heart muscle from the sudden blockage of a coronary artery by a blood clot. Coronary arteries are blood vessels that supply the heart muscle with blood and oxygen. Blockage of a coronary artery deprives the heart muscle of blood and oxygen, causing injury to the heart muscle. Injury to the heart muscle causes chest pain and chest pressure sensation. If blood flow is not restored to the heart muscle within 20 to 40 minutes, irreversible death of the heart muscle will begin to occur. Heart muscle continues to die for six to eight hours at which time the heart attack usually is “complete.” The dead heart muscle is eventually replaced by scar tissue. WHEN A HEART ATTACK OCCURS, NO TIME MUST BE LOST IN GETTING THE PATIENT TO HOSPITAL.
Causes of heart disease
- Atherosclerosis, thrombosis and arteriosclerosis
Two main factors are responsible for heart disease: atherosclerosis (the formation of deposits) and the presence of blood clots (thick blood). However, there is a third problem that can and usually does occur along with atherosclerosis. That is arteriosclerosis, the hardening of the arteries. Arteries are elastic and , whether or not atherosclerosis is present, tend to lose their elasticity and harden with age. One reason for this is a lack of vitamin C, which is needed to make collagen, the intercellular ‘glue’that keeps skin and arteries supple. Arteriosclerosis, atherosclerosis and thick blood can all raise blood pressure, putting you at greater risk of thrombosis, angina, a heart attack or a stroke.
- High blood pressure
In the same way that the pressure in a hosepipe increases and decrease as the tap is turned on and off, the pressure in the arteries increase when the heart beats and decreases in lull before the next beat. These are called your systolic and diastolic blood pressure respectively. If the arteries are blocked, or if the blood is too thick, the pressure increases in order to provide enough blood for the heart. Normal blood pressure is below 120/80; blood pressure between 120/80 and 139/89 is called “pre-hypertension”, and a blood pressure of 140/90 or above is considered high. An elevation of the systolic and/or diastolic blood pressure causes arterosclerosis, which in turn, increases risk of developing heart disease.
- High blood lipid
Hyperlipidemia (high blood lipid) refers to increased levels of lipids (fats) in the blood, including cholesterol and trglycerides. Cholesterol can’t dissolve in the blood. It has to be transported to and from the cells by carriers called lipoproteins. It is combined with low-density lipoprotein, or LDL, it is known as “bad”cholesterol; combined with high-density lipoprotein, HDL, it is known as “good”cholesterol.
LDL (Bad) Cholesterol
If a large proportion of a person’s cholesterol is combined with LDL, it is more likely to be deposited in the artery walls. Together with other substances, it can form plaque, a thick, hard deposit that can narrow the arteries and make them less flexible. This condition is known as atherosclerosis. If a clot forms and blocks a narrowed artery, heart attack or stroke can result.
HDl (good) Cholesterol
HDL takes cholesterol out of the arteries and back to the liver. So it has been popularized as “good” Cholesterol.The ideal ratio is one part HDL cholesterol to three parts total cholesterol (the combination of LDL, HDL and triglycerides level in your blood). Triglycerides
Triglycerides is a form of fat made in the body. Elevated triglycerides can be due to overweight/obesity, physical inactivity, cigarette smoking, excess alcohol consumption and a diet very high in carbohydrates (60 percent of total calories or more). People with high triglycerides often have a high cholesterol level, including a high LDL (bad) level and a low HDL (good) level. Many people with heart disease and /or diabetes also have high triglyceride levels.

Research conducted by VA Medical Center in Providence, Rhode Island, USA; Massachusetts-based Framingham Heart Study, USA; together with a study carried out by the European Concerted Action Group from nineteen medical centers in nine European countries suggested that having a high level of homocysteine in the blood was as great a risk factor for cardiovascular disease as smoking or having elevated blood cholesterol level.
